CSI delivers person-centered services designed to help adults achieve housing stability, improve health outcomes, and build pathways to independence through the Medi-Cal initiative, CalAIM. We serve individuals across Sacramento, Yolo, Solano, and Placer counties.
Housing Support Services
We help individuals and families navigate the path to stable housing through housing navigation, tenancy support, and ongoing assistance to maintain long-term housing success.
Enhanced Care Management
Enhanced Care Management (ECM) is an intensive, whole-person approach to care coordination for individuals with complex health and social needs. Our care managers work closely with members to address barriers, coordinate services, and support long-term health and stability. ECM includes 7 core components:
Rehabilitative Services
We provide personalized support to help individuals build daily living skills, manage health conditions, and work toward greater self-sufficiency. Our rehabilitative programs meet people where they are and focus on practical progress that leads to lasting stability

How do I access CSI’s services through Medi-Cal?

CSI's Enhanced Care Management (ECM) and Community Supports are available at no cost to eligible Medi-Cal members. To access services, you can:
Contact your Medi-Cal Managed Care Plan and request a referral to ECM or Community Supports
Ask your primary care provider, social worker, or case manager to refer you
Call CSI directly at [phone number] and our team will help determine your eligibility and coordinate with your Medi-Cal plan
Connect with us through community partners, hospitals, or social service agencies

Do I need a referral to get started?

While referrals are helpful, they're not always required. You can contact CSI directly, and we'll work with your Medi-Cal Managed Care Plan to determine eligibility and authorize services. Self-referrals are accepted.

Who Eligible for Enhanced Care Management (ECM)?

Who eligible for Community Supports (Housing Deposits, Housing Navigation, etc..)
You may be eligible for ECM if you are a Medi-Cal member with complex health and social needs, including:
Adults experiencing homelessness or housing instability
Adults at high risk for avoidable hospital or emergency department visits
Adults with serious mental health conditions or substance use disorders
Adults transitioning from incarceration
Nursing facility residents transitioning to community living
Homeless families or unaccompanied youth
Your Medi-Cal Managed Care Plan makes the final eligibility determination.
Community Supports are available to Medi-Cal members whose needs cannot be fully met through traditional medical services alone. Eligibility is determined through a comprehensive assessment showing that these services are medically necessary and cost-effective alternatives to hospital or institutional care.
Initial Contact: Our team will reach out within 24-48 hours to introduce ourselves
Comprehensive Assessment: We'll meet with you to understand your health, housing, and social needs
Care Plan Development: Together, we'll create a personalized care plan with your goals
Service Authorization: We'll coordinate with your Medi-Cal plan to authorize services
Ongoing Support: Your Care Manager will work with you to implement your care plan
